Mission Statement - The purpose of the Louisiana College Athletic Training Department is to provide the student-athletes of Louisiana College’s varsity athletic teams the best health care possible. In conjunction with coaches and strength coordinators, we are responsible for the prevention, evaluation, and rehabilitation of athletic injuries, and referral for additional medical treatment if seen necessary by our staff. Through the use of various rehabilitation equipment and techniques along with electrical modalities, thermotherapy, cryotherapy, manual treatment, and sport specific exercises the athletes are provided with an atmosphere that best promotes their overall well being. It is our goal as an Athletic Training staff to offer the highest quality of care while quickly and safely returning our student-athletes to their highest level of competition.
IMPORTANT INFORMATION FOR INCOMING STUDENT ATHLETES
Welcome to Louisiana College Athletics, we look forward to meeting you in the fall. However, before you arrive we need you to take care of some business first. You will need to visit the website below and complete some medical paperwork. It should only take about 20-30 minutes to complete. You will not be able to get a physical without first completing these documents, and without a physical you will not be aloud on the practice field. It is extremely important that you have this completed before your arrival in August.
INSTRUCTIONS: (please read completely before starting so you have all the information ready to fill out)
1. Enter this website: http://active-med.cc
2. Click on “Find Your School” on the left side of the page
3. The password is: wildcats
4. The first page is general demographic information, please fill out as completely as possible with the exception of height and weight, we will do that when you get here.
5. At the bottom of the page are two check boxes for medical and dental insurance. You must have ALL policy information to complete this section. This includes: Policy holder’s address, date of births, social security number. Also, the name, address, group number and policy number of your insurance company.
NOTE: if you can’t fill out this part of the form correctly, please answer NO and bring the information, including the insurance cards, with you.
6. If you answered YES in the previous step, please enter all information on the insurance page and check the appropriate box at the bottom of the page. You will still need to BRING YOUR INSURANCE CARD, or a copy of front and back, with you to LC.
7. The next page is your Medical History. Please answer all questions honestly and to the best of your knowledge.
8. The next page is the Consent for Participation. Please read all statements and check the appropriate box at the bottom of the page.
9. The next page is Louisiana College’s Drug Testing Consent Form. Please read all statements and check the appropriate box.
10. The Last page is Informed Consent of Privacy of Medical Records. Please read all statements and check the appropriate box at the bottom of the page.
Thank you for your time to start the registration process. When you arrive on campus you will learn the time and day of your physicals, BRING YOUR INSURANCE CARDS!!!
